New Patient Registration

Patients can register with the Practice upon production of a valid NI medical card or by filling in the appropriate forms. You must live within the practice boundary area of 4.5 miles

Please contact reception to arrange collection of new patient registration forms. You will also need to provide some form of photo ID and a utility bill with your current address

 

IF THIS IS THE FIRST TIME REGISTEREING WITH A GP IN THE UK AND YOU HAVE MOVING FROM ANY EU COUNTRY YOU MUST COMPLETE THE FORM ON THE FOLLOWING LINK - https://www.gov.uk/settled-status-eu-citizens-families. Your registration can not be processed in the surgery without this.

 

Changes to Personal Details

If you change your name, address or your telephone number please let the surgery have your new details.
